Immediate Loading of Dental Implants in the Anterior and Posterior Mandible: A Retrospective Study of 120 Cases

The objective of this retrospective study was to evaluate the overall success of immediately loaded implants in the mandible. This report focuses on immediately loaded anterior and posterior mandibular implants.
Materials and Methods

A total of 979 patients received 2,904 mandibular dental implants placed from January 2007 to December 2008. One hundred sixty-one of the implants were placed anterior to the mental foramen, and 95 were placed posterior to the mental foramen. All immediately loaded implants were torque tested to 35 N-cm. All were provisionally restored the day of insertion.
Results

A total of 256 immediately loaded mandibular dental implants were placed in a 2-year period. Of the 256 immediately loaded implants, 252 integrated and were restored and 4 failed. Of the 4 failed implants, 3 were in the anterior mandible and the fourth was in the posterior mandible. All 4 were replaced, integrated, and have been restored. The survival rate for immediately loaded implants in the anterior mandible was 99.4%, and the survival rate in the posterior mandible was 97%.
Conclusion

Immediately loaded implants have predictable clinical success in the anterior and posterior mandible.

Young children who consume substantial amounts of fluoride through infant formula and other beverages mixed with fluoridated water or by swallowing fluoride toothpaste have an increased chance of developing mild enamel fluorosis, according to research published in the October issue of The Journal of the American Dental Association and supported by the National Institute of Dental and Craniofacial Research. Children can continue using fluoridated water and fluoride toothpaste because fluoride has been proven to prevent tooth decay, and mild fluorosis does not negatively affect dental health or quality of life.

"Nearly all of the fluorosis in our study participants was mild. A recent review of the effects of mild dental fluorosis on oral health related quality of life concluded that the effect of mild fluorosis was not adverse and could even be favorable," according to the study. "This suggests that concerns about mild dental fluorosis may be exaggerated. Therefore, no general recommendations to avoid use of fluoridated water in reconstituting infant formula are warranted. "

According to the American Dental Association, mild enamel fluorosis appears as barely noticeable faint white lines or streaks on tooth enamel that may occur when children ingest excess fluoride while their teeth are developing.

Researchers concluded that substantial fluoride consumption from beverages with fluoridated water, including infant formula, by children between the ages of 3 to 9 months, elevates a child's prospect of developing mild enamel fluorosis. Substantial fluoride consumption from beverages with fluoridated water and from fluoride toothpaste by children between the ages of 16 to 36 months also elevates a child's probability of developing mild enamel fluorosis.

The American Academy of Pediatrics recommends breastfeeding for infants. If parents are concerned about reducing the chances of their infants developing mild fluorosis through consuming substantial amounts of infant formula mixed with fluoridated water, the researchers suggest that they consult with their family dentist or physician. The researchers also encourage parents to follow recommendations to use small (smear or pea-sized) amounts of fluoridated toothpaste and ensure proper supervision of the child's tooth brushing.

Researchers recruited mothers of newborn infants from eight Iowa hospital postpartum wards between 1992 and 1995 for their children's participation in what would become known as the Iowa Fluoride Study, an investigation of dietary and non-dietary fluoride exposures, dental fluorosis and dental cavities. Researchers sent questionnaires to the parents at regular intervals and 630 children underwent visual dental examinations using standardized techniques and portable equipment. The leader of the research team was Steven M. Levy, D.D.S., M.P.H., the Wright-Bush-Shreves Endowed Professor of Research in the Department of Preventive and Community Dentistry at the University of Iowa's College of Dentistry and Professor of Epidemiology at the College of Public Health.

HPV Causing "Slow Epidemic" of Oral Cancers

October 21, 2010 — Human papillomavirus (HPV) is a risk factor for oropharyngeal squamous cell carcinoma (OSCC), and might account for the steady increase in OSCC incidence, even in subjects who do not smoke or consume alcohol, according to Swedish researchers.
A review of recent studies, conducted by Trobjorn Ramqvist, MD, and Tina Dalianis, MD, PhD, and published online October 13 in Emerging Infectious Diseases, suggests that changes in sexual practices are behind the surge in OSCC cases linked to sexually transmitted HPV. The key factors appear to be multiple sex partners, starting sexual activity at a younger age, and increased oral sex.
The data are startling. For example, from 1970 to 2002, tonsillar cancer (which is the most common OSCC) increased in Stockholm, Sweden, by 2.8-fold, and by 2006/07, 93% of all tonsillar cancers in that city were HPV-positive.
Dr. Dalianis, who is professor of tumor virology and the head of the Department of Oncology–Pathology at Karolinska Institutet in Stockholm, told Medscape Medical News that "we realized that there was an increase in HPV-induced tonsillar cancer, but we did not realize it was so eminent until we separated the 2 groups (HPV-negative and -positive tonsillar cancer cases) the way we did."
The most common OSCC is tonsillar cancer, followed by base of tongue cancer. Overall 5-year survival for OSCC is about 25%, and HPV-positive OSCC generally has better clinical outcomes than HPV-negative disease.
Dr. Dalianis said that HPV (most commonly type 16) was found in 45% to 100% of OSCCs in various studies.
"It was also observed that patients with HPV-positive OSCC were younger and lacked the traditional risk factors of smoking and alcohol consumption," the authors write. "We suggest the increased incidence of OSCC depends on HPV infection and results in an increased proportion of HPV-positive OSCCs."
Dutch Expert Questions Proportions of HPV Positivity
Using data from the Swedish Cancer Registry, the researchers found that the incidence of HPV-positive tonsillar tumors almost doubled over each decade from 1970 to 2007, for a cumulative 7-fold increase over that period.
Similarly, HPV-positive base of tongue cancers increased from 54% in 1998/99 to 84% in 2006/07.
However, another expert, asked to comment on the study by Medscape Medical News, raised questions about the high proportion of HPV-positive tumors found in the Stockholm registry data.
Boudewijn J.M. Braakhuis, PhD, is from the section of tumor biology in the Department of Otolaryngology/Head and Neck Surgery at VU University Medical Center in Amsterdam, the Netherlands. Dr. Braakhuis, whose work focuses on oral and oropharyngeal cancers, said that the proportion of HPV-positive OSCCs in the Amsterdam area is about 20%.
Dr. Braakhuis said that the Swedish analysis is hampered by the lack of a standardized method for measuring HPV involvement. Dr. Braakhuis's group proposes doing p16 staining followed by general primer polymerase chain reaction with typing, which might reduce the risk for false positives.
The Case for HPV Vaccination
Dr. Dalianis said that there is a significant association of HPV-positive tonsillar cancer with early initial sex, and with the number of oral or vaginal sex partners. She also noted that one study reported "that not only oral sex, but also open-mouthed kissing, was associated with the development of oral HPV infection."
Noting that this could mean that oral-to-oral contact might play a role in oral HPV transmission, Dr. Dalianis said that this has implications for the timing of vaccinating children to prevent HPV.
Dr. Dalianis is "a strong supporter of giving the HPV vaccine to both girls and boys," and writes that "it is crucial to monitor the effects of the present HPV vaccination, not only on the incidence of cervical cancer but also on the incidence of OSCC."
Dr. Braakhuis agreed: "One may expect that vaccination will decrease the number of HPV-positive oropharyngeal cancers" after decades.
Another important point uncovered in the review is that the incidence of head and neck squamous cell carcinoma is decreasing and OSCC is increasing, which might presage a risk for overtreatment if the intensified regimens used for head and neck squamous cell carcinoma are applied to HPV-positive OSCC, which has a better prognosis than HPV-negative disease.
"It is possible that increasing numbers of OSCC patients with a better prognosis are being treated with intensified therapy. As a result, many patients have substantial chronic unnecessary side effects. It is therefore necessary to identify which patients need and which do not need intensified treatment," the study authors write. The suggestion is that OSCC patients with HPV-positive tumors who have never smoked might be candidates for less intensive treatment.

So what was new at the 2010 ADA Session in Orlando?- Small Equipment

I am a big believer in magnification and illumination.  So it was great to see a new product from Orascoptic called "Freedom" I have ordered the "Freedom" with 5.0 magnifiers. I cannot wait to try it!

Triodent the makers of the V3 Matrix Rings have released new titanium triple trays. They are so new they are not listed on the web site. I ordered some to try. They are very rigid to prevent distortion and have a disposable mesh. I will let you know how they work out once I get them.

So what was new at the 2010 ADA Session in Orlando?- CarieScan Pro


 As usual there were some interesting products and events taking place. The biggest news was Philips the makers of Sonicare bought Discus Dental. What this means to wet gloved dentist will remain to be seen but for now things will be business as usual for both companies and their products.
Over the next few days I will be discussing some new and updated products that I got to see and play with during the 3 days of exhibits.

 

So today lets look at CarieScan Pro. that comes to us from Scotland and is being sold by Patterson Dental.  It is a simple to use caries detection device that is simple to use and minimizes false positives.  CarieScan Pro uses AC impedance to quantify dental caries.  Don’t worry, as the current is so low that the patient does not feel it.
 The tooth is air dried and the disposable tip is placed onto the tooth. Don’t worry there are prompts to guide you through the process. The CarieScan Pro provides a qualitative value based directly on the disease state, rather than the indirect optical properties of the tooth or surrogate markers such as bacterial breakdown products.  The device beeps when the measurement is complete and a number is displayed on the LCD screen.  It looks like an interesting caries detection device. The only potential downside it that the disposable tips cost $2.00 a piece. I forgot to write down the cost but I believe it was in the $3-4 thousand dollar range.

Net32 today reported that it completed a social technographic survey of members of its industry leading dental comparison shopping marketplace, revealing a number of major differences between dentists’ online behavior and that of the US adult online population. For example, dentists identified themselves most frequently as Critics as defined in the social media focused book Groundswell; people who rate and review products and services, contribute to forums, comment on blogs, etc. About 25% of online US adults are classified as Critics while over 40% of dentist respondents identified themselves as such by their interests and activities.
Dentists were also represented strongly as Collectors; people who receive real-time updates from sites that interest them, tag photos, rate websites, and suggest websites. 26% of the dentists identified as Collectors, more than double the 12% US online adult Collectors. In the category of Creators; people who publish blogs, upload video, write articles, etc, dentists identified themselves as this at a 10% level, below the 18% of online US adults who are classified as such.

Stem cells harvested from mice have been used to reattach molars in rats
Researchers from the University of Illinois at Chicago have successfully used stem cells to reattach molars to tooth sockets within lab rats' mouths. The stem cells were obtained from the periodontal ligament of molars extracted from mice, expanded in an incubator, and then seeded onto barren rat molars. This could have great implications for humans who lose teeth due to gum disease in the future... and for all those toothless old rats out there.
Two to four months after being placed in the rats' mouths, the stem cells on the teeth aligned and formed new fibrous attachments between the tooth and bone. The result was firmly attached teeth. Tissue sections indicated that the replanted teeth were surrounded by newly formed, functional periodontal ligament fibers and new cementum, which are both essential to tooth attachment.

To make sure it was the mouse stem cells that were responsible, they were labeled with green fluorescent protein before being seeded onto the teeth. As a control, other molars were inserted without stem cells - these all either fell back out, or only became loosely attached and ultimately got broken down.
"Our strategy could be used for replanting teeth that were lost due to trauma or as a novel approach for tooth replacement using tooth-shaped replicas," said Tom Diekwisch, director of U Illinois' Brodie Laboratory. The process reminds us of theteeth grown from stem cells, as achieved by Columbia's Dr. Jeremy Mao.
The research was recently published in the journal Tissue Engineering.

I have been using XMarks for syncing my bookmarks between computers. The best thing about Xmarks was the ability to sync across operating systems and different browsers. So it did not matter what you were using. As a Windows/Mac/Linux user this made things very simple. Unfortunately coming in January the service will be shutting down.

Gum Disease Found to be Significant Public Health Concern
Centers for Disease Control and Prevention (CDC) and American Academy of Periodontology (AAP) suggest more American adults may have gum disease; previous estimates underestimate periodontal disease in the US population
September 21, 2010 – Chicago – The prevalence of periodontal disease in the United States may be significantly higher than originally estimated. Research published in the Journal of Dental Research from the Centers for Disease Control and Prevention (CDC) and the American Academy of Periodontology (AAP) suggests that the prevalence of periodontal disease may have been underestimated by as much as 50 percent. The implication is that more American adults may suffer from moderate to severe gum disease than previously thought.
In a National Health and Nutrition Examination Survey (NHANES) pilot study, funded by the CDC’s Division of Oral Health, a full-mouth, comprehensive periodontal examination was conducted on over 450 adults over the age of 35. Periodontal disease was classified according to definitions determined by the CDC in collaboration with the AAP. The prevalence rates were then compared against the results of previous NHANES studies which used a partial-mouth periodontal examination. Historically, NHANES has served as the main source for determining prevalence of periodontal disease in US adults. The pilot study finds that the original partial-mouth study methodology may have underestimated true disease prevalence by up to 50 percent.
Periodontal disease is a chronic inflammatory disease that affects the gum tissue and other structures supporting the teeth. If left untreated, it can lead to tooth loss, and may also interfere with other systems of the body. Several research studies have associated gum disease with other chronic inflammatory diseases such as diabetes, cardiovascular disease, and rheumatoid arthritis.
“This study shows that periodontal disease is a bigger problem than we all thought. It is a call to action for anyone who cares about his or her oral health.” said Samuel Low, DDS, MS, associate dean and professor of periodontology at the University of Florida College of Dentistry, and president of the American Academy of Periodontology. “Given what we know about the relationship between gum disease and other diseases, taking care of your oral health isn’t just about a pretty smile. It has bigger implications for overall health, and is therefore a more significant public health problem.”
Dr. Low explained that the increased prevalence of periodontal disease makes it essential to maintain healthy teeth and gums. “Not only should you take good care of your periodontal health with daily tooth brushing and flossing, you should expect to get a comprehensive periodontal evaluation every year,” he advised. A dental professional, such as a periodontist, a specialist in the diagnosis, treatment and
prevention of gum disease, will conduct the comprehensive exam to assess your periodontal disease status.
According to Paul Eke, MPH, PhD, epidemiologist at the CDC and lead author of the study, the findings have significant public health implications. “The study suggests we have likely underestimated the prevalence of periodontal disease in the adult US population,” he said. “We are currently utilizing a full-mouth periodontal examination in the 2009/10 NHANES to better understand the full extent and characteristics of periodontal disease in our adult population.” Dr. Eke added, “Research suggests a connection between periodontal health and systemic health. In light of these findings, understanding the relationships between periodontal disease and other systemic diseases in the adult U.S population is more crucial than ever.”

Ontario Canada bans health providers from treating spouses or partners

Ontario Court of Appeal decision says ZERO TOLERANCE RULE bans health providers from treating spouses or partners.

The College is advising members, on a go forward basis, to reconsider the wisdom of providing dental treatment to romantic partners and spouses, as a result of an Ontario Court of Appeal decision in February. While the College received a letter in 1995 from the Minister of Health permitting treatment of spouses, that landscape appears to have changed.

The appeal court unanimously ruled that even if a regulated health care provider’s sexual relationship with a partner predated their professional relationship, that provider has committed sexual abuse and risks losing his or her licence for at least five years, if a complaint is filed and there is a referral of specified allegations of professional misconduct to the Discipline Committee.
The court’s decision applies to 26 health professions and the more than 256,000 health care providers in Ontario governed by the Regulated Health Professions Act (RHPA), including dentists.

The zero tolerance rule in the Health Professions Procedural Code,

which is part of the RHPA, stipulates that a health care professional governed by the RHPA who has “sexual intercourse” with a “patient” shall have his/her license revoked for a minimum of five years, if there is a finding by the Discipline Committee following a complaint or an investigation.

According to the appeal court’s judgment, the Code’s provisions do not exempt the health care practitioner from either liability for sexual abuse or from the mandatory penalty, where the health care professional and the patient are having sexual intercourse in the context of a relationship as spouses or a spousal-type relationship.
There is no room for interpretation. The appeal judges went on to state that “when it comes to sexual relations between a doctor and a patient, there is a black letter, bright line prohibition with a drastic sanction and no exceptions or exemptions. The zero tolerance policy precludes inquiry into any explanation or excuse for the sexual activity. A patient’s consent is irrelevant.”

The appeal court recognized that it “may appear that the strict and significant mandatory penalty that follows from a conviction for professional misconduct by
sexual abuse is harsh and arguably unjust in cases where there is a consensual sexual relationship and no exploitation by the health care professional. However, the panel found that the importance of upholding the zero tolerance policy outweighs its pitfalls because the legislation is there to address a growing problem of sexual abuse of patients by some health care professionals.”

The court made a minor exception for what it calls “incidental” care, which covers off situations of emergency care. The court said “it would be unreasonable for a spouse to be denied treatment in such circumstances.”

The court said that if the government wished to provide exceptions it would have done so when it amended the Regulated Health Professions Act in June 2009. The fact that it did not means that it is the law of the land.

This matter arose out of a decision at another College, the College of Chiropractors of Ontario. The issue arises only when there is a formal complaint or formal information leading to an investigation conducted by
the College.

The Green Lounge will include a mini-operatory set-up to demonstrate green dental products in action, such as DentalEZ’s eco-friendly dental chair, Transcendentist®, reusable sterilization pouches, infection control barriers from Discus Dental, and eco-friendly disinfectant from SciCan™. Visitors to the lounge will also be able to pick up instrument recycling kits from
Hu-Friedy’s Environdent® program and engage in hands-on demonstrations.

DentalEZ will showcase its energy-and cost-saving, state-of-the-art EverLight™. “The EverLight is the centerpiece of my eco-friendly operatory,” states Dr. Fred Pockrass, EDA Clinical Director. “Of course I value the energy efficiency, waste reduction, and comfort of working under the EverLight. But what has really impressed me is that the EverLight gives me complete control in the placement of composite restorations without premature setting.”

ADA annual session attendees can pick up a green-colored re-usable stainless steel water bottle from EDA Corporate Member companies and conveniently re-fill it at the Green Lounge’s eco-friendly drink station. “We want to make an impact on the thousands of disposable water bottles that convention-goers use and throw away, and show participants how easy it is to make small changes that make a big difference,” explained EDA Co-Founder, Ina Pockrass. Visitors to the lounge will also get to try Joysence™ hand sanitizer, available from Discus, which helps dental offices reduce their water use while infusing the office with an aroma therapy scent that calms and relaxes patients.

Instead of distributing copious paper brochures, the EDA will hand out eco-friendly bamboo flash-drives loaded with a Special Edition GreenDOC™ Product Guide, listing the industry’s environmentally friendly products and services. The flash drive will include bonus materials, such as educational presentations about green dentistry.

Participants can take a “Green Tour” of the ADA floor and complete a game card from the Green Lounge to be entered to win an iPod Touch.

On Sunday from 3:30 to 5:30 pm, ADA attendees are invited to come to the Green Lounge for a “Green-Tini” cocktail party hosted by Dentaltown.
