Majority of Americans Would Opt to Whiten Their Teeth to Enhance Their Smile

The majority of Americans agree. A recent telephone survey conducted by Harris Interactive, and commissioned by Philips as part of the Get It Right! campaign, found that 89 percent of adults agree that a bright or white smile makes a good first impression and 55 percent believe a person's smile is the best physical indicator of someone's friendliness. Given this, it is no surprise that the survey also found that more than half (53 percent) of adults say if they could enhance one thing about their smile it would be the whiteness of their teeth, more than the straightness of their teeth or any other cosmetic feature.(1)
The survey also revealed that while most adults feel that a smile is important for how a person is perceived, there is a gap between the positive perception towards teeth whitening and the action that people take. Only 20 percent of adults have had their teeth whitened by in-office treatments (2) despite the fact that more than three quarters (77 percent) of adults agree that a white smile is best achieved by seeing a dental professional. Finding further support that 57 percent of adults do not believe over-the-counter (OTC) products are as effective as professional teeth whitening and 29 percent do not believe OTC products work at all.
"Teeth need care and attention like any other part of the body and teeth whitening could play an important role in teeth maintenance," explained Dr. Bill Dorfman, DDS, one of the creators of Zoom whitening and best-selling author. "However, many factors must be considered when whitening a smile, such as teeth sensitivity or a medical condition. No one knows more about oral health than a dental professional who can help advise patients about whitening options, including professional treatments, to ensure safe results for an optimal smile."
There are numerous causes of discolored teeth, including age, genetic factors, food and beverage consumption, tobacco use, poor dental hygiene, disease, medication use and tooth trauma.(3) Given all of the causes of discolored teeth, it is no surprise that teeth whitening has evolved into one of the most popular aesthetic dental treatments. And consumers have many whitening options available to choose from, including toothpastes, whitening strips and professional treatments, that are administered by a dental professional, including a dentist, hygienist or dental assistant, such as light-activated whitening conducted in the dental office.

Abstract

This study evaluated the association between body mass index (BMI) and periodontal condition in a population of Brazilian women. A hospital convenience sample of 594 eligible women was recruited from a women's health reference center of Belo Horizonte, Brazil. Four groups were formed considering BMI levels: BMI normal group (n = 352), overweight (n = 54), obesity level I (n = 48), obesity level II (n = 56), and obesity level III (n = 74). Full-mouth periodontal examination was performed and biological, demographic, and behavioral risk variables were evaluated. Obese and overweight women showed statistically significant differences in bleeding on probing, probing depth and clinical attachment level ≥4 mm, and frequency of periodontitis (p < 0.05) compared to women showing normal BMI. The final multivariate model for the occurrence of periodontitis revealed that obesity groups were significantly associated with periodontitis. In addition, age (25-45), smoking, diabetes, and hypertension remained significantly associated with the occurrence of periodontitis (p < 0.05). Periodontitis was positively associated with obesity, and this association was more evident as obesity levels increases. These findings indicate the need for early diagnosis and the inclusion of periodontal care in health care programs for obese women.

Risk factors and symptoms associated with xerostomia: a cross-sectional study

Abstract

Background:  The aim of this study was to examine the symptoms and risk factors associated with self-reported xerostomia.
Methods:  Data were collected from 601 self-administered questionnaires among dental clinic attendees. Logistic regression models to estimate odds ratios and 95% confidence intervals were used to investigate the association for exposures of interest, such as socio-demographic characteristics, self-reported symptoms, oral hygiene habits and xerostomia.
Results:  Participants reported having dry mouth in 19.6% of cases. Xerostomia was associated with a significant increase in the odds of having dry lips, throat, eye, skin and nose. Patients with self-reported xerostomia were three times more likely to drink water to swallow food than were patients without xerstomia. Older individuals were significantly more likely to report dry mouth, and the prevalence of xerostomia increased with advancing age. The prevalence of xerostomia in patients taking one or more drugs was significantly higher compared to medication-free patients, and increased with increasing numbers of medications used. Finally, individuals with a nervous or mental disorder, or who wore removable dentures were five times more likely to develop xerostomia than patients without disorder or dentures.
Conclusions:  Dentists should be familiar with the symptoms of xerostomia and be prepared to take an active role in the diagnosis, management and treatment of the oral complications.

Comparison between observed children's tooth brushing habits and those reported by mothers

Background

Information bias can occur in epidemiological studies and compromise scientific outcomes, especially when evaluating information given by a patient regarding their own health. The oral habits of children reported by their mothers are commonly used to evaluate tooth brushing practices and to estimate fluoride intake by children. The aim of the present study was to compare observed tooth-brushing habits of young children using fluoridated toothpaste with those reported by mothers.

Methods

A sample of 201 mothers and their children (aged 24-48 months) from Montes Claros, Brazil, took part in a cross-sectional study. At day-care centres, the mothers answered a self-administered questionnaire on their child's tooth-brushing habits. The structured questionnaire had six items with two to three possible answers. An appointment was then made with each mother/child pair at day-care centres. The participants were asked to demonstrate the tooth-brushing practice as usually performed at home. A trained examiner observed and documented the procedure. Observed tooth brushing and that reported by mothers were compared for overall agreement using Cohen's Kappa coefficient and the McNemar test.

Results

Cohen's Kappa values comparing mothers' reports and tooth brushing observed by the examiner ranged from poor-to-good (0.00-0.75). There were statistically significant differences between observed tooth brushing habits and those reported by mothers (p < 0.001). When observed by the examiner, the frequencies of dentifrice dispersed on all bristles (35.9%), children who brushed their teeth alone (33.8%) and those who did not rinse their mouths during brushing (42.0%) were higher than those reported by the mothers (12.1%, 18.9% and 6.5%, respectively; p < 0.001).

Conclusions

In general, there was low agreement between observed tooth brushing and mothers' reports. Moreover, the different methods of estimation resulted in differences in the frequencies of tooth brushing habits, indicative of reporting bias. Data regarding children's tooth-brushing habits as reported by mothers should be considered with caution in epidemiological surveys on fluoridated dentifrice use and the risk of dental fluorosis.

A joint approach to treating dental phobia: a re-evaluation of a collaboration between community dental services and specialist psychotherapy services ten years on

Objective To audit the records of a group of patients who had previously benefited from cognitive behavioural therapy (CBT) for dental phobia.
Aim To ascertain if they had returned to the use of intravenous (IV) sedation to facilitate dental treatment. Ten years ago these patients were routinely requiring IV sedation to facilitate dental treatment due to severe dental phobia.
Method Sixty patients entered the original pilot project. Of those, 30 were offered CBT and 21 attended. Twenty of those patients (95.2%) were subsequently able to have dental treatment without IV sedation. In this follow-up study the electronic records of 19 of the 20 patients who had originally been successful with CBT were re-audited. Our purpose was to see if there was any record of subsequent IV sedation administration in the intervening ten years.
Results Of the 19 successful CBT patients available to follow-up, 100% had not received IV sedation since the study ten years ago. This may suggest the initial benefit of CBT has endured over the ten-year period.
Conclusion This study indicates that the use of CBT for patients with dental phobia proves beneficial not only in the initial treatment but that the benefits may endure over time. This results in a significant reduction in health risks to the patient from repeated IV sedation. It may also translate into significant financial savings for dental care providers. Our evidence for CBT as treatment for dental phobia suggests dental services should be implementing this approach now rather than pursuing further research.

Significance of molecular markers in survival prediction of oral squamous cell carcinoma

 Abstract

Background

An accurate system for predicting the survival of patients with oral squamous cell carcinoma (OSCC) will be useful for deciding appropriate therapies. The prediction accuracy of prediction models can be improved by using molecular biomarkers. We constructed a nomogram for predicting the survival of patients with OSCC using clinical variables and molecular markers.

Methods

Protein 53 (p53), insulin-like growth factor II mRNA-binding protein 3 (IMP3), cyclooxygenase 2 (COX2), and HuR were localized immunohistochemistry in 96 patients with primary OSCC who underwent surgical resection between January 1994 and June 2003 at the Yonsei Dental Hospital in Seoul, Korea.

Results

On univariate and multivariate analysis, the expression of IMP3 was significantly associated with the risk of death. P53 was also significantly associated with survival of OSCC in the case of negative IMP3 and the prediction accuracy was improved by including these 2 factors in the prediction model.

Conclusion

Survival in OSCC can be predicted more accurately by using biomarkers. The constructed nomogram predicted survival after treatment for an individual patient with OSCC, and it can be practically used as a tool to help decide which adjuvant treatment is most appropriate. © 2011 Wiley Periodicals, Inc. Healthy Smiles For Autism


BALTIMORE, MD (September 16, 2011)—The National Museum of Dentistry has created a new resource to help parents of children with autism spectrum disorder succeed in teaching good oral healthcare. Healthy Smiles for Autism is a guide that helps parents teach children with autism spectrum disorder how to brush and floss with the help of step-by-step instruction, social stories, and visual sequencing cards that can be used wherever brushing happens. This guide is free and downloadable at www.healthysmilesforautism.org.

The Healthy Smiles for Autism guide was created to empower parents of children with autism spectrum disorder with usable tools to help effectively teach their children an oral health routine. The guide also provides information to help parents prepare their children for a first dental visit. 

“We want to be able to give parents readily usable tools to help their children to develop a good oral hygiene regimen,” said National Museum of Dentistry Executive Director Jonathan Landers. “We’ve combined best practices for autism education, such as visual sequencing cards and rewards systems, with proven personal oral hygiene techniques to help make the process a little bit easier.”

The National Museum of Dentistry partnered with Kennedy Krieger Institute’s Center for Autism and Related Disorders and University of Maryland Dental School to develop these best practices to oral health care for children with autism spectrum disorder.

Autism is a complex developmental disability that typically appears during the first three years of life and affects a person’s ability to communicate and interact with others – including the dentist. Dental care is the leading unmet healthcare need among children with special needs, and across all income levels, children with special needs are almost twice as likely to have an unmet oral heath care need than their peers without special needs, according to the National Maternal and Child Oral Health Resource Center.

Triotray Pro- My Favorite Triple Tray


 I have been using prototypes of these triple trays and was thrilled to finally get the final product, Triodent's Triotray Pro. It makes taking impressions easier and more accurate. MY laboratory likes them because they can easily do multiple pours of the impression.

Top 5 dental queries revealed

Patients are most in the dark about implants, crown and bridges when it comes to dentistry.
That's according to new figures released today (Tuesday) by the British Dental Health Foundation.
Implants, crowns and bridges and other removable appliances topped the list of queries flooding into its countrywide dental helpline, making up around one in seven (13%) of all enquiries. 
Increasingly important issues to the public included advice on oral hygiene (7%) and legal and professional conduct issues (6%).
The findings were based on analysis of more than 150,000 enquiries from the public over the past five years and offered an insight into some of the trends over that time.
In 2006, five issues accounted for well over half of all enquiries from the public (58%). 
These included NHS Regulations (15%), Dental Charges (13%), Finding a Dentist (11%), Prosthetics (11%) and Complaints (8%).
In 2011, the same five issues accounted for 44% of all enquiries – but there were significantly lower enquiries relating to dental charges (-8%), NHS Regulations (-4%) and Finding a Dentist (-3%).
Sharon Broom, the Foundation's director of operations, says: 'A new dental contract in 2006 created significant interest in NHS regulations and charges. Since then, the public has become more familiar with NHS Dentistry. However, with a new dental contract being piloted in some parts of the UK, we are already forecasting another increase in calls when the new contract is rolled out.
'Overall, the public has remained fairly consistent with its information needs over the past five years. We are pleased that the number of calls regarding oral hygiene is increasing. A greater awareness of consumer rights in all aspects of everyday life is also spreading into dentistry, with more and more callers seeking information on legal issues.'
'The Foundation's Dental Helpline is staffed by qualified dental nurses and costs, on average, £9 per call. It continues to be funded entirely by charitable donations and we hope the dental profession and trade continue to support this unique and important part of patient communication.'

Abstract

PURPOSE:

To evaluate the severity of post-radiation dental lesions and possible correlation with radiation dose to the teeth in patients treated for head and neck cancers.

METHODS AND MATERIALS:

Data from 93 head and neck radiotherapy patients treated between 1997 and 2008 were analyzed retrospectively. The main effect, radiation dose to the individual teeth, was evaluated with covariates of elapsed time after radiation, xerostomia, topical fluoride use, and oral hygiene status included. Patients' radiotherapy plans were used to calculate cumulative exposure for each tooth. Patients' teeth were evaluated using a validated index and then categorized as having none/slight or moderate/severe post-radiation damage.

RESULTS:

Patients (31 females, 62 males) ranged in age from 18-82 yrs (mean=57). The number of teeth/patient ranged from 3-30 (mean=20) with a total of 1873 teeth evaluated. Overall, 51% of teeth had moderate/severe damage, with the remaining having little or none. Using odds ratios and 95% confidence intervals, the odds for moderate/severe damage were 2-3x greater for teeth exposed to between 30-60 Gy as compared to no radiation. However, for teeth exposed to ≥60 Gy as compared to no radiation the odds of moderate/severe tooth damage was greater by a magnitude of 10 times.

CONCLUSIONS:

The results indicate that there is minimal tooth damage below 30 Gy (salivary gland threshold), a greater than 1:1 increased dose-response between 30-60y likely related to salivary gland damage, and a critical threshold of ≥60Gy which may be linked to direct effects of radiation on tooth structure. These findings suggest that care should be taken during the treatment planning process to limit tooth dose, and when clinically possible to limit tooth dose to less than 60 Gy.

Reliability of Logicon caries detector in the detection and depth assessment of dental caries: An in-vitro study

Background: Digital radiography has so far not resulted in improved rates of proximal caries detection. Historically, automated caries detection tools have been largely academic. Opinions regarding the performance of the only such commercially available tool, viz., Logicon caries Detector (LCD) have been equivocal. This study was conducted to evaluate the reliability of LCD in the detection and depth assessment of proximal caries.
Materials and Methods: Digital images were obtained of 100 proximal tooth surfaces using the Kodak RVG 5000 sensor and analyzed by three observers. The images were then analyzed by the principal investigator using the LCD software. The teeth were then sectioned and magnified photographic images were obtained which were taken as the gold standard. All the grades were entered in proformas and the data were statistically analyzed using the chi-square test. Five parameters of reliability were calculated.
Results: The sensitivity, specificity, positive predictive value, negative predictive value, and overall accuracy of LCD for the grade No caries were 33, 96, 73, 82, and 81%, respectively; for the grade Enamel caries were 5, 97, 33, 80, and 79%, respectively; and for the grade Dentin caries were 100, 96, 50, 100, and 96%, respectively.
Conclusions: In conclusion, LCD appears to be more reliable in ruling out (both enamel and dentin) caries than in detecting caries.

Effect of ProRoot MTA, Portland cement, and amalgam on the expression of fibronectin, collagen I, and TGFβ by human periodontal ligament fibroblasts in vitro

Context: Today many materials have been introduced for root-end filling materials. One of them is mineral trioxide aggregate (MTA) that is mentioned as a gold standard.
Aims: The purpose of this in vitro study was to evaluate the reaction of human periodontal ligament fibroblasts to the root-end filling materials, such as ProRoot MTA, Portland cement, and amalgam.
Settings and Design: Eight impacted teeth were extracted in aseptic condition. The tissues around the roots were used to obtain fibroblast cells. After cell proliferation, they were cultured in the chamber slides and the extracts of the materials were added to the wells.
Materials and Methods: Immunocytochemical method for measuring the expression of Fibronectin, collagen I and transforming growth factor beta (TGF®) was performed by Olysia Bioreport Imaging Software.
Statistical Analysis Used: The results were analyzed by SPSS 13.0 and Tukey post hoc test with P<0.05 as the limit of significance.
Results: Collagen expression in MTA specimens was higher than the other groups in 24 h significantly. After 48 h, the Portland cement group showed the most expression of collagen significantly and after 1 week, Portland cement and MTA groups had the most expression of collagen but there was no significant difference between these 2 groups. After 1 week, the Portland cement group demonstrated a higher amount of TGF® and fibronectin.
Conclusions: The results suggest that Portland cement can be used as a less expensive root filling material with low toxicity. It has better effects than amalgam on the fibroblasts.

Hurricane Irene and My Dental Office

My home and office survived Hurricane Irene without any big physical plant issues. The big issue for my office was lack of power. We had to cancel patients on the Monday after the hurricane as the lower level of my building had flooded and there was no power. I know many dental offices are in worse shape, some have been destroyed and others will have no power for up to a week. I learned a few things besides having redundant backups both on and off site.

What happens if I have no power at my home and office? Make sure you have a notebook computer and a way to recharge the battery. A power inverter for use in the car can be used for recharging the battery. Thanks to Lighthouse PLZ my office manager was able to access a copy of  our appointment book from her home and cancel patients for Monday. We called Officite our web site company and had theme post a big banner announcing the office was closed on our web site. We changed the voicemail message to indicate were were not in and we would return calls the next day.  Keep copies of the install discs for you practice management software in case you need to reload the software along with your backup data.  Internet access is becoming more crucial so have a smart phone which you can use to tether to a computer or or a USB cellular modem.

With hurricanes you have time to prepare. Some emergencies give you no time. So make sure you have an emergency plan for your office in place.
