With an ample amount of time during the bus ride to Piscataway, guests were treated to educational videos about Colgate’s history and were also given a light-hearted Jeopardy-like quiz comprised of simple questions about the surrounding tri-state area, dental history and holiday fun facts, as well as industry-specific questions that only savvy dental professionals would know.
Upon arrival at Colgate’s Global Research & Technology Center guests were greeted by Colgate experts and treated to a number of informative presentations. All guests were able to thoroughly examine relevant current topics such as evidence-based dentistry, microbiology, triclosan safety, periodontal therapy, implants and implant maintenance, and much more. 

The educational Destination Innovation Discovery Bus Tour was free of charge and all guests received 3 CE credits. In addition, complimentary lunch was provided for each tour.

Moreover, new insights on implant maintenance and the role of triclosan with respect to inflammation were thoroughly examined as well as a review of the safety and efficacy of triclosan for use in Colgate Total® toothpaste.

JADA Study Finds No Association Between Elevated Blood Sugar And Implant Failure.


Medscape (12/11, Harrison) reports that according to a study published in the December issue of the Journal of the American Dental Association “dentists can safely place dental implants in patients with uncontrolled diabetes,” which “contradicts common recommendations.” Said the study’s author Thomas W. Oates Jr., DMD, PhD, “We failed to identify any association between elevated blood sugar and implant failure or implant complications.” Medscape points out that although previous studies have found implants to be less stable in patients with uncontrolled diabetes, those studies “were of short duration.” In this study, “examiners blinded to the diabetes status of the patients assessed the implants during a 4-month healing period after placement, and then again at 3, 6, and 12 months after implant restoration,” and found that “implant survival rates a year after restoration were not statistically different” for those without diabetes, with “well-controlled” diabetes, or for those with “poorly-controlled” diabetes.

Erosive cola-based drinks affect the bonding to enamel surface: an in vitro study

Objective:
This study aimed to assess the impact of in vitro erosion provoked by different cola-based drinks (Coke types), associated or not with toothbrushing, to bonding to enamel.
Material and methods:
Fifty-six bovine enamel specimens were prepared and randomly assigned into seven groups (N=8): C- Control (neither eroded nor abraded), ERO-RC: 3x/1-minute immersion in Regular Coke (RC), ERO-LC: 3x/1-minute immersion in Light Coke (LC), ERO-ZC: 3x/1-minute immersion in Zero Coke (ZC) and three other eroded groups, subsequently abraded for 1-minute toothbrushing (EROAB-RC, EROAB-LC and EROAB-ZC, respectively). After challenges, they were stored overnight in artificial saliva for a total of 24 hours and restored with Adper Single Bond 2/Filtek Z350. Buildup coronal surfaces were cut in 1 mm2 -specimens and subjected to a microtensile test. Data were statistically analyzed by two-way ANOVA/Bonferroni tests (α=0.05). Failure modes were assessed by optical microscopy (X40). The Interface of the restorations were observed using Confocal Laser Scanning Microscopy (CLSM).
Results:
All tested cola-based drinks significantly reduced the bond strength, which was also observed in the analyses of interfaces. Toothbrushing did not have any impact on the bond strength. CLSM showed that except for Zero Coke, all eroded specimens resulted in irregular hybrid layer formation.
Conclusions:
All cola-based drinks reduced the bond strength. Different patterns of hybrid layers were obtained revealing their impact, except for ZC.

Does the number of implants have any relation with peri-implant disease?

Objective:
The aim of this study was to evaluate the relationship between the number of pillar implants of implant-supported fixed prostheses and the prevalence of periimplant disease.
Material and Methods:
Clinical and radiographic data were obtained for the evaluation. The sample consisted of 32 patients with implant-supported fixed prostheses in function for at least one year. A total of 161 implants were evaluated. Two groups were formed according to the number of implants: G1) ≤5 implants and G2) >5 implants. Data collection included modified plaque index (MPi), bleeding on probing (BOP), probing depth (PD), width of keratinized mucosa (KM) and radiographic bone loss (BL). Clinical and radiographic data were grouped for each implant in order to conduct the diagnosis of mucositis or peri-implantitis.
Results:
Clinical parameters were compared between groups using Student’s t test for numeric variables (KM, PD and BL) and Mann-Whitney test for categorical variables (MPi and BOP). KM and BL showed statistically significant differences between both groups (p<0 .001="" 19="" 26="" compared="" differences="" from="" g1="" g2="" implants="" of="" p="0.0210).</p" peri-implantitis="" prevalence="" regarding="" showed="" significant="" statistically="" the="" with="">
Conclusion:
It seems that more than 5 implants in total fixed rehabilitations increase bone loss and consequently the prevalence of implants with periimplantitis. Notwithstanding, the number of implants does not have any influence on the prevalence of mucositis.

Occlusal splint vs. modified NTI splint in bruxism therapy. Randomized, controlled trial using surface electromyography

 

Abstract

Background

An occlusal splint and a modified nociceptive trigeminal inhibition splint (AMPS, anterior deprogrammer, Kois deprogrammer, Lucia jig, etc.) are commonly and quite frequently used in the treatment of masticatory muscle disorders, although their sustainable and long-lasting effect on these muscles’ function is still not very well known. Results of scant surface electromyography studies in patients with temporomandibular disorders have been contradictory.

Objective

To evaluate both devices in bruxism therapy, EMG activity levels during postural activity and maximum voluntary contraction of the superficial temporal and masseter muscles were compared before and after 30 days of treatment.

Study Design

Surface electromyography of the examined muscles was performed in two groups of bruxers (15 patients each). The patients in the first group used occlusal splints, while those in the second used modified nociceptive trigeminal inhibition splints. The trial was randomised, controlled, and semi-blind.

Results

The use of neither device affects the asymmetry index or postural activity/maximum voluntary contraction ratio after one month of treatment.

Conclusions

Neither the occlusal nor the nociceptive trigeminal inhibition splint showed any significant influence on the examined muscles. The use of different scientific methods should be considered in clinical applications that require either direct influence on the muscles’ bioelectrical activity or a quantitative measurement of the treatment quality.

Dentinal Crack Formation during Root Canal Preparations by the Twisted File Adaptive, ProTaper Next, ProTaper Universal, and WaveOne Instruments

Abstract

Introduction

The aim of this study was to compare the incidence of root cracks after root canal instrumentation with the TF Adaptive, WaveOne, ProTaper Next, and ProTaper Universal systems.

Methods

Results

The control group had no cracks, and the difference between the control group and the experimental groups was statistically significant (P < .001). The ProTaper Next and TF Adaptive systems produced significantly less cracks than the ProTaper Universal and WaveOne systems in the apical section (3 mm) (P < .05).

Conclusions

Under the study conditions and within the limitations of this study, it can be concluded that the ProTaper Universal, ProTaper Next, WaveOne, and TF Adaptive instruments can result in dentinal cracks.

Analysis of professional malpractice claims in implant dentistry in Italy from insurance company technical reports, 2006 to 2010.

Abstract

PURPOSE:

The aim of the study was to analyze the characteristics of implant dentistry claims in Italy based on insurance company technical reports for malpractice claims.

MATERIALS AND METHODS:

One hundred twenty-one technical reports of cases of professional malpractice in implant dentistry between 2006 and 2010 were included in the study. Data included the sex and age of the patient and dentist, the kind of negligence claimed, and the damages awarded as a consequence of the alleged misconduct.

RESULTS:

Of the cases examined in this study, 9.9% went to court. The patients were female in 73.6% of the cases. Most of the technical errors were committed during implant insertion (82.6%). In 50.4% of cases, the technical error involved the surrounding structures, such as damage to the inferior alveolar nerve (32.2%) or the lingual nerve (2.5%), invasion of the maxillary sinus (9.1%), or pulpal dental necrosis in adjacent teeth (6.6%). Incomplete clinical documentation was apparent in 54.5% of cases. In 9.9% of cases, a civil suit had already been filed before a visit, and medicolegal advice from the insurance expert had been procured.

CONCLUSION:

The discrepancy between the total number of cases examined and those that went to court indicates that implant malpractice claims in Italy are most often settled out of court. The large number of intraoperative errors seen and the high proportion of injuries to surrounding structures suggest that implant dentists would benefit from further specific training. Also, clinical documentation vital to a defense against any claims relating to professional misconduct was incomplete or absent in more than half of the cases.

Analysis of 415 adverse events in dental practice in Spain from 2000 to 2010.

Abstract

INTRODUCTION:

The effort to increase patient safety has become one of the main focal points of all health care professions, despite the fact that, in the field of dentistry, initiatives have come late and been less ambitious. The main objective of patient safety is to avoid preventable adverse events to the greatest extent possible and to limit the negative consequences of those which are unpreventable. Therefore, it is essential to ascertain what adverse events occur in each dental care activity in order to study them in-depth and propose measures for prevention.

OBJECTIVES:

To ascertain the characteristics of the adverse events which originate from dental care, to classify them in accordance with type and origin, to determine their causes and consequences, and to detect the factors which facilitated their occurrence.

MATERIAL AND METHOD:

This study includes the general data from the series of adverse dental vents of the Spanish Observatory for Dental Patient Safety (OESPO) after the study and analysis of 4,149 legal claims (both in and out of court) based on dental malpractice from the years of 2000 to 2010 in Spain.

RESULTS:

Implant treatments, endodontics and oral surgery display the highest frequencies of adverse events in this series (25.5%, 20.7% and 20.4% respectively). Likewise, according to the results, up to 44.3% of the adverse events which took place were due to predictable and preventable errors and complications.

CONCLUSION:

A very significant percentage were due to foreseeable and preventable errors and complications that should not have occurred.

Do bonding agents protect the bracket-periphery?—Evaluation by consecutive μCT scans and fluorescence measurements

Abstract

Objectives

The aim of this in vitro study was to consecutively determine the effect of three bonding agents on the prevention of enamel demineralisation at the bracket-periphery and to compare the suitability of micro-computed tomography (μCT) scans and quantitative light-induced fluorescence (QLF) to detect changes within subsurface lesions.

Materials and methods

The effect of a resin-modified glass ionomer cement (RMGI) (Fuji Ortho LC), a compomer (Assure) and a composite (Transbond XT) on the prevention of enamel demineralisation at the bracket-periphery was examined. After 7, 14, 21 and 28 days of pH cycling, the teeth (N = 45) were examined by consecutive μCT scans and by using a customised QLF set-up.

Results

Particularly for the RMGI and for the compomer, the QLF and μCT scans showed that the formation and the body of the lesion were not precisely located at the enamel next to the bracket margin. There was an area that was almost protected. The progression of demineralisation was decreased for the RMGI and the compomer-treated teeth.

Conclusion

For bonding orthodontic brackets, the RMGI and compomer were comparably able to decrease the progression of white spot lesions (WSL), although the RMGI showed marginally superior protection. Both methods (QLF and μCT scans) were suitable for investigating the longitudinal fluoride effects on WSL, though these effects were more accurately described by mineral (fluorescence) loss or volume changes than by lesion depth.

Clinical relevance

The progression of WSL at the bracket-periphery could be altered by using fluoride-releasing bonding agents for bracket application. This approach represents a minimally invasive preventive measure.
