OSAP Provides Critical Resources for Swine Flu Epidemic

Dental Educators Meet With Members Of Congress To Advocate For Universal Dental Coverage And Access To Oral Health Care

Advocates for dental education and dental research participated in the 2009 American Association for Dental Research-American Dental Education Association (AADR-ADEA) Advocacy Day on Capitol Hill yesterday. Representatives from dental schools visited 36 Members of Congress to advocate for universal coverage and access to oral health care in U.S. health care system reform. To achieve that goal, AADR-ADEA advocates urged Congress to make needed investments in dental education to ensure a robust, diverse health professions workforce. They also urged congressional leaders to support a 10% increase in the FY 2010 budget for the National Institutes of Health (NIH) and the National Institute of Dental and Craniofacial Research (NIDCR).

"Grassroots advocacy by our members is an essential component of ADEA's legislative efforts. Raising the profile on Capitol Hill of dental education and research while health care reform is a priority on the agenda of national lawmakers is key to improving access to oral health care," said ADEA President Ronald J. Hunt, D.D.S.

Participants attended a one-day legislative workshop that included presentations by nationally known columnist and commentator Bill Press; frequent radio and television news guest and digital budget columnist for Stanley Collender; NIH Acting Principal Deputy Director Lawrence A. Tabak, D.D.S., Ph.D.; and senior policy advisor at the Washington, DC law firm of Alston & Bird, LLP and former Clinton White House staffer Marilyn Yager. Before their legislative visits, participants were briefed by staff from the ADEA Center for Public Policy and Advocacy (ADEA CPPA) and AADR on each of the advocacy messages they would deliver to their elected officials.

AADR-ADEA Advocacy Day is an annual event designed to enhance the legislative leadership skills of dental educators and researchers while advocating for the legislative priorities of both associations. Over the years, participants have educated Congress about issues including the State Children's Health Insurance Program (SCHIP), the Children's Dental Health Improvement Act, and restoration of funding for student aid programs. Each year, dental school deans, faculty and administrators, allied dental educators and professionals, dental researchers, patient advocate groups, and ADEA Volunteer Advocacy Coordinators from around the country attend the event.

Prevalence of malocclusion among mouth breathing children: Do expectations meet reality?

The aim of this study was to report epidemiological data on the prevalence of malocclusion among a group of children, consecutively admitted at a referral mouth breathing otorhinolaryngological (ENT) center. We assessed the association between the severity of the obstruction by adenoids/tonsils hyperplasia or the presence of allergic rhinitis and the prevalence of class II malocclusion, anterior open bite and posterior crossbite.

Cross-sectional, descriptive study, carried out at an Outpatient Clinic for Mouth-Breathers. Dental inter-arch relationship and nasal obstructive variables were diagnosed and the appropriate cross-tabulations were done.

Four hundred and one patients were included. Mean age was 6 years and 6 months (S.D.: 2 years and 7 months), ranging from 2 to 12 years. All subjects were evaluated by otorhinolaryngologists to confirm mouth breathing. Adenoid/tonsil obstruction was detected in 71.8% of this sample, regardless of the presence of rhinitis. Allergic rhinitis alone was found in 18.7% of the children. Non-obstructive mouth breathing was diagnosed in 9.5% of this sample. Posterior crossbite was detected in almost 30% of the children during primary and mixed dentitions and 48% in permanent dentition. During mixed and permanent dentitions, anterior open bite and class II malocclusion were highly prevalent. More than 50% of the mouth breathing children carried a normal inter-arch relationship in the sagital, transversal and vertical planes. Univariate analysis showed no significant association between the type of the obstruction (adenoids/tonsils obstructive hyperplasia or the presence of allergic rhinitis) and malocclusions (class II, anterior open bite and posterior crossbite).

The prevalence of posterior crossbite is higher in mouth breathing children than in the general population. During mixed and permanent dentitions, anterior open bite and class II malocclusion were more likely to be present in mouth breathers. Although more children showed these malocclusions, most mouth breathing children evaluated in this study did not match the expected “mouth breathing dental stereotype”. In this population of mouth breathing children, the obstructive size of adenoids or tonsils and the presence of rhinitis were not risk factors to the development of class II malocclusion, anterior open bite or posterior crossbite.

Dropbox-Sync files between computers!

I have been using Dropbox for a few weeks now and really like this application. It allows you to sync files between computers. It works no matter what operating system you are using Windows, Mac or Linux. So if I am working on a Word document on my Windows PC at work I can place it in my Dropbox folder and it is upload to the Dropbox server. When I get home and turn on my Macbook the Word document will automatically download to my Mac.

Baush & Lomb to acquire Steri-Oss

Bausch & Lomb Inc. of Rochester said yesterday that it planned to buy Steri-Oss, a maker of dental implants based in Anaheim, Calif. The value of the all-cash deal was not disclosed. The annual dental-implant market is about $100 million in the United States and $150 million to $160 million in Europe, according to Bausch & Lomb, which expects the market to grow by about 20 percent annually. Steri-Oss, founded in 1985, also makes surgical instruments used to insert the implants into a patient's mouth and other implant-related products. Steri-Oss has 100 employees and annual sales of about $13 million.

Rise in children admitted to hospital with rotting teeth

Experts have described the findings as indicative of a "major public health issue".

Up to 30,000 children a year are admitted to hospital for dental work with 80 per cent of cases involving extractions.

Tooth decay accounts for more than half of all extractions in children aged 17 and under. The figure has increased by 66 per cent since 1997 according to the report published by the British Dental Journal.

The majority of extractions involve the patient being given a general anaesthetic, which can be dangerous to children.

The study noted that children from poorer backgrounds were particularly at risk, being twice as likely to need treatment as those from more affluent areas.

Update on Healthcare Credit Line

My office has still been trying to find out what will happen to monies that we are due and were not paid to us. We have been contacted by a few different dental offices seeing if we found out anything and no one has heard back from Healthcare Credit Line. We have asked the patients to contact Healthcare Credit Line. Because many have put off further treatment or want to stop making payments to the lender. No patient has gotten back to us so far with any additional information.

Here is what we currently do know after my office manager spoke with someone in the legal department at the ADA.

It looks like we will be screwed. There are two agreements one between the patient and Healthcare Credit Line and one between the dental office and Helathcare Credit Line. The patients are legally obligated to make payments even though we have not been paid for our services. We do not know if we can claim non payment and the patient can stop making payments. We were further instructed to consult an attorney in our state to see exactly what remedies maybe available to us.

As we find out more I will keep everyone reading the blog in the loop.

ClearCorrect Voluntarily Dismisses Lawsuit Against Invisalign®

From the outset, ClearCorrect has made it clear that if Align provided assurance that it did not intend to sue ClearCorrect, then ClearCorrect would voluntarily dismiss the case. In its moving papers, Align has represented to the Court that:

"(Align) has never expressly or impliedly asserted any claim or engaged in any conduct that indicated that it believed (ClearCorrect) infringed any claim under any patent or that it would engage in any action as a result of such infringement."

In addition, Align has expressly disavowed any statements of its employees that indicated Align intended to sue ClearCorrect. Align has further stated that no employee has been authorized to make such statements. Align has also represented that it has not engaged in any "scare the customer" tactics.

Align has clarified that there is no black cloud of impending litigation. ClearCorrect's principals can now concentrate on growing their business. More on RED FLAG rules from the ADA

Please help the ADA help you by writing to your Representatives in Congress, asking them to contact the Federal Trade Commission (FTC) to express opposition to the Federal Trade Commission's plan to apply the Red Flags Rule on identity theft to dental practices. The Rule is currently scheduled to go into effect on May 1.

As you know, more than 80% of dental practices comprise one or two dentists and identity theft is generally not a problem for dental offices. This rule would therefore have very little benefit, but would add unnecessary costs to the delivery of dental care. The American Dental Association estimates that the average cost to implement and manage a Red Flags program will be at least $600 per dental office.

It is very possible that compelling dentists to comply with the Rule could lead to serious conflicts with HIPAA privacy provisions and thus result in additional costs associated with obtaining legal advice. Unfortunately, despite all this compelling evidence, which I and ADA staff have explained in direct conversations with the FTC, the Commission still persists in its position that the Rule applies to virtually every dental office in the nation regardless of size.

I am pleased to report, however, that we have had some recent progress. Because of ADA efforts, the Chairwoman of the House Small Business Committee recently sent a letter to the FTC Commissioner asking for a delay in the implementation of the Red Flags Rule, as well as issuance of a revised rule that clarifies whether health professionals face any obligations to implement a Red Flags program. In her letter, the Chairwoman clearly stated that in her considered opinion, the FTC failed to follow legally mandated procedures. She said the FTC did not conduct a meaningful analysis of the effect the Rule would have on healthcare providers, and the FTC also failed to provide an opportunity for healthcare providers to submit comments about the burdens the Rule would place on them (the letter is attached here)

I am also attaching here the April 6 ADA News article about what the ADA has already done on your behalf to fight the Red Flags Rule. Now we need your help to keep the ball rolling. Communicate to your elected representatives the undeniable fact that forcing dental practices to comply with the Red Flags Rule will have little impact on identity theft, do nothing to improve the oral healthcare of the public, and cost dentists money and resources that should be expended elsewhere, especially in these economically distressed times.

Tooth decay cure may be added to sweets

DENTAL scientists yesterday claimed to have discovered how to prevent the bacteria that cause tooth decay from forming in the mouth.
BASF, the world's largest chemical manufacturer, said the formula could be introduced into toothpaste, mouthwash and even sweets by next year.

Using a micro-organism related to those used in yogurt cultures, the German-based company has engineered a process that clusters harmful bacteria in the mouth before they can bind with sugar and form plaque.

The organism and plaque-causing bacteria are then swallowed as part of the mouth's natural cleaning process.

The bacteria, known by the scientific name Streptococcus mutans, is safe to swallow because it is naturally found in the mouth.

"There is a complex dynamic of bacteria that grow on the teeth," said Jason Tanzer, of the University of Connecticut.

"They can stick to those surfaces and form a film or they can be swallowed."

Scientists fed two groups of rats a diet high in sugar, but put BASF's product in only one group's food.

Tooth decay in the group of rats using the product was far less pronounced than in the group not using the product.

Consistent evidence to support the use of xylitol- and sorbitol-containing chewing gum to prevent dental caries

Studies were identified using searches with Medline, the Cochrane Library and Google Scholar.
Study selection

Studies were screened independently and were included if they evaluated the effect of one or more chewing gums containing at least one polyol (xylitol, sorbitol, mannitol or maltitol) on caries development, provided they supplied original data generated by means of a comparative design (experimental or observational) and were published in English. Studies were excluded if only an abstract was available or they described only the pharmacodynamic or pharmacokinetic properties of polyols or did not include a no-treatment arm in the study. Randomised trial quality was assessed using the Jadad scale, and the US Preventive Services Task Force criteria to grade the internal validity of individual nonrandomised studies.
Data extraction and synthesis

Data were extracted independently with only the final outcomes of a study being recorded. It was decided that surface rather than tooth level data would be recorded. Incremental caries was converted to prevented fraction (PF; the proportional reduction in dental caries in experimental groups relative to control groups) for meta-analysis. The studies were grouped according to type of polyol and a separate meta-analysis performed. Data were pooled using both a random and a fixed-effects model and heterogeneity assessed using I2.

Of 231 articles identified 25 studies were initially selected with 19 being included in the review [six randomised controlled trials (RCT) of which four were cluster RCT, nine controlled clinical trials (CCT) and four cohort studies]. Two RCT had a Jadad score of three or higher. The mean preventive fraction for the four main gum types are shown in the table 1, results of all except the sorbitol -mannitol blend were statistically significant. Sensitivity analyses confirmed the robustness of the findings.

Although research gaps exist, particularly on optimal dosing and relative polyol efficacy, there is consistent evidence to support the use of xylitol- and sorbitol-containing chewing gum as part of normal oral hygiene to prevent dental caries.

Researchers Discover Why Teeth Form in a Single Row

In 1975, Steven Spielberg ruined America’s summer vacation with the release of his man-versus-shark classic Jaws. Universal Pictures promoted Spielberg’s blockbuster in cinema lobbies with a signature poster featuring the word JAWS in blood red, an innocent young woman swimming in the water below, and the open mouth of a gigantic shark rising to the surface with sinister intentions. To highlight the gruesome attack to come, the artist accurately depicted the shark as possessing multiple rows of teeth. This artistic flourish has led many a spooked moviegoer to wonder then and now: Why do most mammals manage fine with a single row of teeth, while sharks and other creatures of the deep look like they’re chewing barbed wire?

In the February 27 issue of the journal Science, NIDCR grantees reported they may have solved the mystery. Teeth normally form early in embryonic development from a well-recognized mode of tissue induction in which a cell layer called the mesenchyme interacts with another known as the epithelium. This dynamic give-and-take activates the needed genetic programs within each layer to precisely pattern a tooth. Orchestrating the process are DNA-binding proteins called transcription factors. They determine which genes get turned on and off. In the current paper, the scientists found in mice that the transcription factor Osr2 restricts the spatial field in which a tooth can form in the mesenchyme. Remove Osr2, and extra teeth form adjacent to the molars. The scientists found that when present, Osr2 counters the field-expanding actions of the Msx1-Bmp4 pathway in mesenchyme. “In mammals, Osr2 suppresses this pathway along the buccolingual axis [straight across from cheek to tongue] to restrict molar development to one tooth row,” the researchers concluded. “Diversity in dentition patterns in other vertebrates is likely due, at least in part, to evolutionary changes in antagonistic interactions regulating this pathway across the tooth morphogenetic field.”
